Answer:	I doubt it. When some splashed on my sweater while refilling the pen, it washed out easily. Of course, I washed it immediately, but if it was permanent ink, I doubt it would have washed out as easily as it did.

Answer:	No. The neck of the bottle is wide enough to lower your pen inside and refill. There is a silicone stopper inside the top of the neck of the bottle. The neck of the bottle is approximately ½” tall. I had no problem accessing the ink to fill my pen.
