We have had 12 year olds who have a smaller head and mouth prefer the youth and 7/8 year old who have bigger than average heads/mouths that need the adult. After they are custom fit per the instructions you will also get a much better fit, and if you need to get an adult size you can trim that size down in the length too. For me, my son has used it since February 1, 2016 and it is still usable. What happens is, the hook side of the Velcro will only adhere to a small part of the loop side that results in a lesser adhesive strength.
