Answer:	I have never had an issue with it spilling out. I'd assume if that is happening then the bottle is filled to high to warm in a bottle warmer.

Answer:	There is a nipple on each bottle. Love these glass bottles so much faster to warm up I set bottle in hot water to warm rakes two minutes. Easy to sanitize and clean as well.
