Answer:	Yes, it does look like that photo. I had to do minimal assemble, comes in 3 pieces, screw base to long section and long section comes in two parts that slide together. It is 'battery operated' but right next to the item on Amazon is an option to purchase (SEPARATELY) a wall plug adapter and we did that, which to me, should be included with this fairly expensive scale. it is a nice scale and works very well, seems to be very accurate in the short time we have owned it.

Answer:	One of ours rests on flat berber exam carpet and is fine. Wouldn't recommend on any other non-hard surface.
