I think the nipple that the bottle comes with the bottle will be fine for your 4 month old. The Avent nipples come in four flows. New born flow will have a 1 on the nipple--one pin hole at the tip and rated for newborns. Slow flow will have a 2 on the nipple--two pin holes at the tip and rated for 1 month and older. Medium flow will have 3 on the nipple--three pin holes at the tip and rated for 3 months and older. And Fast flow will have 4 on the nipple--4 pin holes at the tip and rated for 6 months and older. The 8 ounce bottle comes with the Slow flow nipple, or #2, rated for 1 month and older babies. I think it is fine for your 4 month old. But each baby varies. You should try the nipple that it comes with first and see how your baby takes to it. If he is drinking fine, then you're ok. If he cries or seems impatient, then you need a faster nipple--move up to the Medium or #3. If he is choking and gasping, then you need to get the Newborn flow or #1.
