Answer: I only use bottled ink, I don't know if ink cartridges work.

Answer: The pen cap falls off and the pen is, IMO, kind of heavy in the hand.

Answer: I've not heard this problem. Perhaps try re-inking it or trying a different ink. Also, did you rinse out the pen before inking it? That may be the problem. Another thing to check is if the capillary action is working properly. This can be solved by re-inking to make sure there are no bubbles in your ink stopping the capillary action. If none of these work then maybe send it back? Twsbi pens are always reliable.
