Some of them has the back piece that crews off and the earring goes through from the front. Some has the gems or the balls screw off from the front and push earring through from the back and the put the gem or ball on. The backs that screw on are male(external) threaded and the post you screw them into are female.(internal) i have 9 piercings. I used 4 of these. One piercing got really sore and itchy. The other three were fine. Been wearing one of these for bout 2 months now. The one that caused my ear to get sore turned blackish. Tarnished. I figured I got a bad one or maybe I snagged it on something. They are cute.
